Keep the FILE OF LIFE Form in your Kitchen

Another way to ensure the best care during a medical emergency is to have a simple document, called a File of Life, easily accessible in your kitchen.  It lists valuable information, such as medications, medical conditions, allergies and emergency contacts, which can be very helpful to first responders.

Free File of Life forms, which can be completed in less than half an hour, are available at EMS headquarters at 182 South Avenue.
